Prescription Pads
We will print your custom designed prescription form on security paper that meets or exceeds the Department of Health (DOH) requirements for counterfeit-proof prescription pads and blanks. We also offer 2 part carbon-less prescription pads with security paper that meets DOH standards. In addition, if you want pads on non-security paper for non-controlled substance prescriptions we would be happy to provide those at well.
DOH requires that the practitioner must submit a signed order in writing. Included is our Order Form so you can write a simple signed order request on your letterhead and fax the request with the order form and sample prescription form to 239-278-0090
